experience. During this initial visit, you will receive an orthodontic
examination by Dr. Hendrix. At that time, we will
be able to determine three things:
1. What type of treatment, if any, is recommended
2. Estimated length of orthodontic treatment
3. Professional fee and payment options
Please call or e-mail our office to schedule an appointment. For your
convenience, we will send you a welcome packet, appointment card,
new patient information sheet, and a confidential medical-dental history
form. Please complete these simple forms and bring them with you to
your first appointment.
